Few clinical chemistry analyses have evoked more debate than serum prostate-specific antigen (PSA). “To screen or not to screen” has been a hot topic for 35 years. Although urology and patient organisations typically are for screening, most healthcare authorities are against. In recent years, Sweden has introduced programmes that, at first glance, resemble organised screening, despite the official recommendation against a national screening programme. What is happening in Sweden and why? In the following, we will shed light on the unique concept of “organised prostate cancer testing” (OPT).
