Ceramides (Cers), a class of bioactive sphingolipids implicated in vascular inflammation, apoptosis, and plaque instability, have emerged as promising biomarkers of cardiovascular risk. Growing evidence supports the prognostic value of circulating Cers and their incorporation into lipidomic risk scores, particularly the Cardiovascular Event Risk Tests 1 and 2 (CERT1 and CERT2), which integrate selected ceramide and phosphatidylcholine species. This review summarizes recent advances in Cers and CERT risk scores.
